Mountain Land Rehabilitation Home Health is an agency focusing on what we do best: rehabilitation. We specialize in services for Physical Therapy, Occupational Therapy, Speech & Language Therapy, and Nursing to ensure coordination between rehabilitation team members. What we offer is unique, patients recovering from home can receive nursing and personal care services along with rehabilitation services right at home.
Mountain Land Rehabilitation Home Health believes that through skilled & caring services provided by compassionate professional & allied caregivers, patients can reach their optimal functional abilities while being cared for in their own home. Each patient is treated with dignity, respect and compassion.
Mountain Land Home Health provides a collaborative approach to the patients in their care. The following home health care providers are utilized in an interdisciplinary approach:
Physicians prescribe and oversee patient plans of care.
Registered Nurses assess patient needs and provide skilled services including: medication instruction, wound care, injections, disease management and education, anti-coagulation management, and case management for rehabilitation needs.
Physical Therapists assess patients’ mobility, strength, and function while setting goals with patients to improve their functional abilities. Physical Therapists provide skilled therapy services including: exercise, mobilization, modalities (heat, cold, ultra sound, etc.) training to improve balance, walking and transfers, and patient education.
Occupational therapists help patients improve performance in activities of daily living such as dressing, meal preparation, feeding, and bathing.
SPEECH LANGUAGE PATHOLOGY
Speech Language Pathologists help patients improve communication, voice use, swallowing and cognition.
HOME HEALTH AIDES (CNA/HHA)
Under the direction of a skilled nurse or therapist, aides assist patients with activities of daily living such as bathing, showering, dressing, toileting, walking and exercise.
Meet Our Staff
Randy Clark, PT, GCS
Home Health Administrator
Randy was born and raised in Salt Lake City, Utah and received his Physical Therapy degree from the University of Utah in 1993. He has been a part of the Mountain Land team since 2011 and became the administrator of Mountain Land’s Home Health Agency in 2012.
As a physical therapist, his clinical focus has been in Home Health and Inpatient Rehabilitation. In 2012 Randy became a Board Certified Geriatric Specialist. Currently there are less than five therapists practicing in Utah who are recognized by the American Physical Therapy Association as Geriatric Certified Specialists.
Randy’s treatment philosophy is to provide accurate information, allowing his patients to make an educated decision on how to approach their injury or disability. He believes in using a variety of techniques that allow for the best outcome without unnecessary pain or discomfort. His ultimate satisfaction is to help someone from being nearly bed bound to no longer being home bound.
In his time away from treating patients, Randy enjoys training for and competing in Olympic distance triathlons, mountain bike races and distance running. He also spends his weekends coaching his boys’ soccer team or watching them compete in national level taekwondo.
Alison S. Coonrod, RN
Director of Clinical Services
Alison is originally from Phoenix, Arizona, and graduated from Glendale Community College in 1983 with her ADN. She moved to Utah in 1996 and has been with Mountain Land since 2012.
She has been practicing as an RN since 1983 and has specialized in gerontology. She has additional training in chemotherapy, wound care, diabetes, rheumatology, and has taken courses in alternative medicine.
Alison truly feels a calling for nursing and loves being able to interact and help people through challenging events in their life. Her nursing philosophy is to connect with her patients on a personal and professional level to aid in their wellness.
In her personal time Alison enjoys baseball, reading, cooking, camping, fishing, listing to classical and jazz music, and her dogs.
