Tyler Ellis, PT, DPT, OCS

Clinic Director, Physical Therapist

Tyler Joined the Mountain Land team in the spring of 2013. He earned his Doctorate of Physical Therapy at Northern Arizona University in 2013 and received his undergraduate degree in Exercise Science from Utah State University in 2010.

Tyler has a strong background in health, fitness and sports performance. While getting his undergrad he played collegiate sports. Tyler threw shot put as member of the university’s track and field team. This experience introduced him to physical therapy and how beneficial it can be in improving performance and injury recovery.

When not in the clinic helping patients get back to full mobility or enhancing their athletic performance, Tyler can be found reading about Neurological Rehab. On more adventurous days he can be spotted outside running, hiking or snowboarding.


Sarah Kettner Physical Therapist

Sarah Johncock, PT, DPT

Physical Therapist

As a native of Michigan, Sarah received her Bachelor’s degree in psychology from the University of Michigan in 2009, then her Doctorate in Physical Therapy from Grand Valley State University in 2012. In addition to her degrees, Sarah is certified in concussion management and has received extensive training in vestibular physical therapy and mechanical diagnosis and therapy as well.

Sarah’s passion is helping patients achieve the highest level of function possible. Sarah has a special interest in concussions and traumatic brain injuries and helping those with vestibular dysfunction. She also works with patients who experience back and neck disorders using a mixture of principles including the McKenzie method.

When she’s not in the clinic, Sarah enjoys hiking, camping, and traveling to new places with her husband and daughter.


Maddy Banbury Physical Therapist

Maddy Banbury, PT, DPT, CF-L1

Physical Therapist

Maddy graduated from Brigham Young University-Hawaii in 2018 with a bachelor’s degree in exercise and sport science. She went on to earn her doctorate of physical therapy from Northern Arizona University at the Phoenix Biomedical Campus in 2021. Maddy is pelvic floor certified through Herman and Wallace, has training in PNF (proprioceptive neuromuscular facilitation) and holds a CrossFit level 1 certification.

Maddy specializes in treating athletes as well as a variety of musculoskeletal, pelvic floor and neurological conditions, and has participated in medical missions to Jamaica in physical therapy. As a prior athlete and physical therapy patient, Maddy knows first-hand the importance of having a physical therapist who is encouraging and supportive. When working with her patients, she believes in using her education and training to bridge the gap between physical therapy, chronic illness and overall health and well-being.

When not in the clinic, Maddy enjoys spending time exploring the outdoors, traveling, doing CrossFit and competing in Olympic weightlifting.


Chelsea Sambuchi Physical Therapist AssistantChelsea Sambuchi, PTA

Physical Therapist Assistant

Chelsea earned her Physical Therapist Assistant degree from the Salt Lake Community College in 2017 and has been practicing physical therapy since graduation.

Specializing in shoulder, neck, and spine injuries as well as balance and fall prevention, Chelsea utilizes functional movements geared to increase strength, mobility and coordination to maximize treatment and prevent re-injury. She also has experience working with medically fragile patients and people with Parkinson’s disease. Chelsea is passionate about finding the right combination of techniques for each individual that will empower them to move through their world as freely and with as little pain as possible.

When not in the clinic, Chelsea enjoys gardening, running, bike riding, swimming, hiking, camping, live music and good food all while spending as much time as she can with her husband and kiddo!
