Mountain Land Physical Therapy West Jordan

The Mountain Land Physical Therapy West Jordan Clinic provides comprehensive physical therapy and rehabilitation services to West Jordan and the surrounding communities. Originally located on the Salt Lake Community College Campus, the West Jordan Clinic is now in a stand-alone facility that enables us to provide state-of-the-art care to all of our patients.

Some features of our new facility include new cardio, exercise and modality equipment, pneumatic unweighting over the treadmill, high speed video analysis of running gait and mechanics, and an open floor layout with ample space for exercise/functional training. We also offer many specialty programs including ASTYM, Hand Therapy and Custom Splint Fabrication, Pediatric OT Programs, Lymphedema Therapy, and Running Rehab.

You will enjoy the clinic’s fantastic view of the Wasatch Mountains as well as your time spent with a fantastic staff, including two board certified clinicians. Our providers pride themselves on excellent patient care and building lasting relationships with their clients. This comes in great part from our patients being able to see the same therapist on each visit throughout their course of care. Scott Maw

Scott B. Maw, PT, MPT, OCS

Clinic Director, Physical Therapist, Board Certified Orthopedic Specialist

Scott joined the Mountain Land team in 2012 but has been practicing physical therapy since graduating from the University of Utah in 2001. He was born in Utah, but grew up moving around the country in a military family.

Scott is a board certified orthopedic specialist through the APTA, and particularly enjoys treating shoulder injuries, both conservatively and post surgically. He believes physical therapy should be fun and focused on goals that are meaningful to each patient.

Scott is a huge sports fan, particularly of college football, baseball, and soccer. He also enjoys running and cross training. For the last few years, Scott has been coaching and assisting his kids’ sports teams and loves to watch them play. Scott’s brother, Sam Maw, is also a physical therapist with Mountain Land at the Salt Lake Clinic.


Deanna Clark

Deanna Clark, OTR, CHT, CLT

Occupational Therapist, Certified Hand Therapist, Certified Lymphedema Therapist

Deanna has been part of the Mountain Land team since 2005, and brings with her over 20 years of experience as an Occupational Therapist specializing in the upper extremities. She graduated from the University of Puget Sound in 1991 with her degree in Occupational Therapy and became a Certified Hand Therapist 1997. In 2011 she also became certified in treating Lymphedema.

Deanna has received extensive training in all aspects of the upper extremities including specific treatment for the thumb, wrist, elbow, and shoulder. She also specializes in custom splinting and myofascial release. She is experienced in evaluating and treating patients with cumulative trauma disorders, post-surgical conditions, traumatic hand injuries, and pediatric motor and writing difficulties.

Sam Maw, PT, DPT

Physical Therapist

Sam grew up in an Air Force Family and has lived all over the country, but has been a resident of Utah since 1995. He received both his Bachelor’s in Exercise & Sport Science and Doctorate of Physical Therapy from the University of Utah.

Sam joined Mountain Land in 2012. He enjoys treating orthopedic and sports medicine patients and believes in combining a hands-on approach with therapeutic exercise. He works with all patient populations and particularly enjoys working with runners. His goal in providing therapy is to help others get back to doing the things that they love.

When Sam is not in the clinic he is busy backpacking, camping, running, traveling, and spending time with his wife and sons.
